Powering Care Coordination Through Mobility

Chronic diseases account for $3 of every $4 spent on healthcare. Chronic diseases such as diabetes, cancer, and heart disease are the leading causes of disability and death in the US, and they cause 7 out of every 10 deaths. http://www.cdc.gov/chronicdisease/index.htm. There are a number of different efforts as part of healthcare reform to include HITECH and ACA, which are introducing new models of care (PCMH, ACO, etc) to address the healthcare crisis. While the models differ, fundamental to these models are care coordination and patient engagement. We can no longer provide episodic care, and we must have a team-based approach for coordinating and managing all of the patients' care.
